眼窩膿瘍の5例

Five cases of orbital abscess

　眼窩膿瘍5例の臨床および画像所見について報告した。臨床所見では眼瞼腫脹，眼痛，眼球偏位が全例に認められ，これらが同時に存在する場合，眼窩膿瘍を疑う有力な所見と思われた。画像所見としては，①全例に副鼻腔炎と眼窩壁の骨破壊像を認めた。②眼窩内膿瘍は境界不明瞭な類円形を示すのに対し，骨膜下膿瘍は境界明瞭な扁平形を示した。またmass effectの存在は眼窩蜂窩織炎との鑑別上重要である。③5例中2例では造影CTで，ring enhanceがみられた。

　抗生物質全身投与で改善を認めない場合，または臨床所見や画像所見から膿瘍が疑われた症例では積極的な手術を施行するべきであると考えた。

We evaluated the clinical and radiological features in 5 cases of orbital abscess. All cases showed eyelid swelling, ocular pain and deviation. Computed tomography (CT) showed following findings. All cases showed destruction of bony orbit associated with parasinusitis. Orbital abscess appeared as uncapsulated round mass and could be differentiated from subperiosteal abscess which showed well-defined and semilunar mass. Above features were also decisive in ruling out orbital cellulitis. Enhanced CT showed ring enhancement in 2 cases. All the 5 cases underwent surgical drainage with success. We advocate this procedure when the condition fails to respond to systemic antibiotic therapy.
