白内障手術により生じる角膜知覚低下を,嚢外摘出術(ECCE),強角膜切開後縫合した超音波乳化吸引術(PEA),自己閉鎖創を用いたPEAについて検討した。対象はそれぞれ15例16眼,15例18眼,7例9眼であった。角膜知覚はDraeger角膜知覚計とCochet-Bonnet角膜知覚計を用い,術後1週,3か月,6か月,1年後に測定した。測定部位は角膜中央部,角膜周辺部(3,6,9,12時)とした。
角膜知覚の低下は,自己閉鎖創によるPEA,強角膜切開によるPEA,ECCEの順に高度となった。角膜知覚低下の範囲は,切開創から角膜中央部に至る扇型の分布を示し,自己閉鎖創によるPEA群では不明瞭な分布となった。
We measured the corneal sensitivity after cataract surgery using two esthesiometers by Draeger and Cochet-Bonnet. The surgical procedure comprised extracapsular cataract extraction (ECCE) with corneoscleral incision 16 eyes, phacoemulsification aspiration (PEA) with sutured corneoscleral incision 18 eyes, and PEA with self-sealing incision 9 eyes. Measurements were taken at the center and along four meridians at a point between the limbus and the center 7 days, 3 months, 6 months and 1 year postoper-atively. The data were processed through a polynomial regression formula. The decrease in corneal sensitivity was most pronounced in ECCE cases and was the least in self-sealing PEA ones. Generally, the profile of the affected area involved a corneal sector enclosed by the arc of limbal incision. This feature was absent in self-sealing PEA cases.
