We evaluated the therapeutic outcome in 64 eyes with massive vitreous hemorrhage complicating diabetic retinopathy. In the first group of 39 eyes, the patient underwent absolute bedrest in the sitting position till the vitreous hemorrhage subsided. Thereafter, we treated the eyes with panretinal photocoagulation.Additional vitrectomy was neces-sary in 21 eyes. In the seconnd group of 25 eyes, we treated the patients with conventional medication and later by vitrectomy.

The final clinical outcome was significantly bet-ter in the first group. Spontaneous lasting cure was obtained in 18 eyes (46%). In 21 eyes that were treated by vitrectomy, surgery was a success in all the cases. Final visual acuity of 0.5 or over was obtained in 17 eyes(86%). At an overall evaluation, the eyes in the first group were characterized by significantly lesser fibrous proliferation. Absolute bedrest in the sitting position appeared to have protected the posterior fundus by inducing the vit-reous hemorrhage to gravitate inferior vitreous cavity.
