網膜下に濃厚な滲出斑を生じたサルコイドーシスの1例

A case of ocular sarcoidosis with massive subretinal exudation

　強い網膜下滲出斑のあるサルコイドーシスの1症例を経験した。症例は59歳の女性。主訴は左眼飛蚊症。初診時，左眼の前房中および硝子体中に軽度の細胞浸潤が認められ，左眼眼底は鼻下側の赤道部から周辺部に，コーツ病類似の黄白色滲出斑と網膜血管の拡張と蛇行が観察された。螢光眼底造影所見では，左眼網膜血管からの著しい螢光漏出が鼻下側の滲出性病変に一致して認められた。全身所見では，アンギオテンシン変換酵素の上昇と，経気管支肺生検において非乾酪性類上皮細胞肉芽腫の所見が得られ，サルコイドーシスと診断した。サルコイドーシスの眼病変は種々の病態を呈する場合があるので，非定型的な症例でもサルコイドーシスの存在を念頭におく必要があると考えられた。

A 59-year-old female presented with floating spots in the left eye since 1 week before. The left eye showed 1+cells in the anterior chamber and vitreous. Funduscopy showed dilated retinal veins and whitish-yellow exudates simulating Coats' disease in the inferior nasal quadrant in the affected eye. Fluorescein angiogaphy showed dye leakage from affected retinal vessels. Sarcoidosis was diagnosed by elevated level of serum angiotensin converting enzyme and noncaseating epitheloid tubercles in the lung. The present case illustrates that ocular manifestations may be variable in ocular sarcoidosis and may develop subretinal exudates.
