精神分裂病患者の角膜内皮細胞

Corneal endothelial cells in schizophrenic patients

　筆者らは，白内障手術を希望する長期罹病の精神分裂病患者3人について，スペキュラーマイクロスコープによる角膜内皮細胞の検索を行った。3例中2例は細胞密度と六角形細胞率の減少が認められたが，1例はほぼ正常の内皮細胞であった。3例とも角膜浮腫は認められなかった。精神分裂病患者における角膜内皮障害の原因は，現在のところ不明である。2症例に対して当科にて白内障手術を施行した。術前から内皮細胞数の減少がある者では，眼内手術時の角膜内皮への侵襲をできる限り少なくすることが重要である。

We examined the corneal endothelial cells by specular microscopy in 3 patients with schizophre-nia preliminary to cataract surgery. The patients were aged 48, 84 and 62 years. Schizophrenia had been present for 26, 30 and 40 years respectively. Corneal edema was absent in all the eyes. The first two patients showed a decrease in endothelial cell density and in the rate of hexagonal cells. The third patient showed almost normal endothelial cell appearance. We failed to identify the exact cause of endothelial damage in our patients. Two patients underwent eventual cataract surgery. We advocate particular care to avoid corneal endothelial damage during cataract surgery for patients with decreased endothelial cell density.
