涙嚢鼻腔吻合術後の組織型プラスミノーゲンアクチベーターによる涙道洗浄

Irrigation of nasolacrimal duct with tissue-type plasminogen activator following dacryocystorhinostomy

　鼻涙管閉塞に対する涙嚢鼻腔吻合術（DCR）後，涙点からの通水が不通であった2症例に，吻合部閉塞を解除する目的で組織型プラスミノーゲンアクチベーター（tPA）による涙道洗浄を行った。tPAを涙点から注入し，60分後に再度通水した結果，鼻腔への疎通が得られた。tPAを用いた通水は，DCR後の疎通不良症例での涙道洗浄として有用であった。また，本剤で吻合部の疎通が得られたことから，DCR後早期の吻合部閉塞には術中，術後の出血が関与していると考えられた。

Two patients underwent dacryocystorhinostomy, DCR. Attempted irrigation of the nasolacrimal passage with saline failed at day 3 and 5 of surgery respectively. We then irrigated the nasolacrimal passage with a solution containing tissue-type plasminogen activator (tPA). One hour later, irrigation with saline passed into the nasal cavity with bloody rhinorrhea. It appeared that intra- and postoperative hemorrhage obstructed the lacrimal duct after surgery and that irrigation with tPA facilitated passage of lacrimal drainage system.
