日帰り計画的水晶体囊外摘出術の術後検討

Post-operative evaluation of outpatient cataract surgery by employing planned extracapsular cataract extraction and intraocular lens implantation

日帰り手術として計画的水晶体囊外摘出術を29例34眼に行った。水晶体核の硬化が強い男性8例9眼，女性21例25眼で，年齢は51～86歳，平均72歳である。術後3か月で0.5以上の視力が83％に得られた。術後の円柱レンズの度数が－3D以上のものが76％であった。術中合併症として，チン小帯断裂が2眼（6％）にあり，術後合併症として術創哆開と前房出血が核1眼（3％）に生じた。日帰り手術として計画的水晶体囊外摘出術で12％の症例に術中・術後合併症があったが，最終的に全例で視力が改善した。

We performed planned extracapsular cataract extraction as day surgery on 34 eyes of 29 patients. The series comprised 9 eyes of 8 males and 25 eyes of 21 females. Their ages ranged from 51 to 86 years,average 72 years. All the cases had relatively hard lens nucleus. Visual acuity of 0.5 or over was obtained in 83％ of eyes three months after surgery. Astigmatism of －3 diopters or greater was present in 76％. Rupture of zonules occurred intraoperatively in 2 eyes（6％）. Loosening of corneoscleral wound and hyphema occurred postoperatively in 1 eye each（3％）. In spite of 12％ of intra- or postoperative complications,visual acuity improved in all the eyes after planned extracapsular cataract extraction as day surgery.
