原発性シェーグレン症候群の涙液ラクトフェリン

Lactoferrin level in the tear fluid in Sjögren's syndrome

　確定診断された原発性シェーグレン症候群50例を対象としてラクトプレート^®を用いて涙液ラクトフェリン濃度を測定した。平均値は1.08±0.86mg／mlで角結膜ローズベンガル染色スコアとの間に相関係数−0.6929と強い負の相関を認めた。涙腺生検が行われた2眼において涙液ラクトフェリン濃度は涙腺組織の状態をよく反映していた。原発性シェーグレン症候群のドライアイ診断におけるラクトプレート^®の感度は72.2％，特異性は78.6％，偽陽性率は21,4％，偽陰性率は27.8％であった。ラクトプレート^®による涙液ラクトフェリン濃度の測定は原発性シェーグレン症候群のドライアイ診断に有用である。

We quantitated the lactoferrin concentration in the tear fluid in 50 patients of definite primary Sjögren's syndrome. We used Lactoplate^TM in the quantitation. The concentration averaged 1.08± 0.86 mg/ml. There was a marked negative correla-tion between lactoferrin concentration and RoseBengal staining scores (r=-0.6929). In two biop-sy specimens, tear lactoferrin concentration well reflected the pathological features in the lacrimal gland. The present method showed sensitivity of 72.2%, specificity of 78.6%, false positivive of 21.4% and false negative of 27.8% in the diagnosis of dry eye in primary Sjögren's syndrome. The findings show the usefulness of quantitation of tear lactoferrin with the present method in the diagnosis of dry eye in primary Sjögren's syndrome.
