The interference pattern of the tear film is mainly caused by its lipid layer derived from the Meibomian glands. With thickening of the lipid layer, the normal network pattern of the tear film changes to severe interference pattern. We examined the tear film pattern in 109 normal and 82 dry eyes using a noncontact specular microscope. We could classify the finding into 4 grades: 1) normal network pattern, 2) more reflex and yellowish color, 3) focal interference pattern and 4) whole interference pattern. When measured by Schirmer test 1, the tear volume in grade 4 eyes was significantly less than in grade 1 and 2 eyes (p<0.05) . It appeared that the decrease in tear volume is associated with the decrease in tear turnover rates, resulting in thickening of lipid layer and appearance of interference pattern.
