Meibomian secretions and tears

　涙液の脂質層が厚くなると，涙液最表層の網目状構造は消失し，干渉色が観察される。今回筆者らは，正常者56例109眼およびドライアイ患者41例82眼の涙液を非接触スペキュラーマイクロスコープを用いて観察し，涙液最表層の状態と涙液量の関係について検討した。涙液最表層は脂質層の状態により4段階に分類し，涙液量はシルマーテスト第1法を用いて測定した。干渉色が著しいgrade 4の涙液量は，網目状構造のgrad 1および反射光量が増加し色調が黄色味を帯びるgrade 2の涙液量に比べて有意に低下していた（p＜0.05）。涙液量が低下すると，正常な涙液交換が行われなくなり，涙液全体に占める脂質層の割合が増加すると考えられる。

The interference pattern of the tear film is mainly caused by its lipid layer derived from the Meibomian glands. With thickening of the lipid layer, the normal network pattern of the tear film changes to severe interference pattern. We examined the tear film pattern in 109 normal and 82 dry eyes using a noncontact specular microscope. We could classify the finding into 4 grades: 1) normal network pattern, 2) more reflex and yellowish color, 3) focal interference pattern and 4) whole interference pattern. When measured by Schirmer test 1, the tear volume in grade 4 eyes was significantly less than in grade 1 and 2 eyes (p＜0.05) . It appeared that the decrease in tear volume is associated with the decrease in tear turnover rates, resulting in thickening of lipid layer and appearance of interference pattern.
