We evaluated 124 patients of Sjögren's syndrome with or without dry eye and mouth. Particular attention was paid to autoantibodies and salivary gland histology in regard to clinical manifestations. The focus score of minor salivary gland was higher in definite dry eye cases than in those with probable or absent dry eye (p<0.05). Presence of anti-SS-A antibody was significantly associated with dry eye complications (p<0.05). Positivity of anti-SS-A and anti-SS-B antibodies increased parallel with severity of dry eye in definite dry eye cases (p<0.05) . There was a proportional relationship in the clinical stages between dry eye and dry mouth. Dry eye tended to be severe in cases with positive autoantibodies. Histological features of salivary glands and autoantibodies need due attention in the diagnosis and treatment of dry eye in Sjögren's syndrome.
