眼内レンズ挿入術前後の瞳孔径の変化

Pupil size before and after intraocular lens implantation

　眼内レンズ挿入術前後の症例44例58眼につき，術前，術後1か月，術後3か月に暗所での瞳孔径，および0.5％トロピカミドと0.5％塩酸フェニレフリン混合液（ミドリンP^®）点眼後の瞳孔径を測定した。暗所での瞳孔径，ミドリンP^®の点眼後の瞳孔径ともに術前に比し，術後1か月では有意に小さく，術後3か月では有意な差がなかった。術後3か月の暗所での瞳孔径と術前の暗所での瞳孔径との相関の方が，術後3か月の暗所での瞳孔径と術前のミドリンP^®点眼後の瞳孔径との相関よりも強く，術後の暗所での瞳孔径を推定する手段として術前の暗所での瞳孔径を測定する方法が優れていると考えられた。術前に暗所での瞳孔径を測定する方法は，白内障手術予定の患者に適切な光学径の眼内レンズを選択する際に有用であると考えられた。

We measured the pupil diameter before and aftercataract surgery with intraocular lens implantationin 58 eyes.

Measurement was performed in darkness or afterinstillation of parasympathicolytic mydriaticagent.

At one month after surgery, the pupil size wassignificantly smaller both in darkness and aftermydriasis than prior to surgery. At 3 months aftersurgery, there was no difference in pupil size fromthe presurgical level.

There was a higher correlation between the pupilsize before and 3 months after surgery in darknessthan after mydriasis. Measurement of the pupil sizein darkness would serve as a guide in selectingproper size of intraocular lens prior to surgery.
