局所麻酔下眼科手術におけるアトロピン前処置の適否

Adequacy of systemic atropine as premedication to ophthalmic surgery under local anesthesia

　局所麻酔下眼科手術における前投薬に，アトロピンを使用した場合と，使用しなかった場合との差異を，ホルター心電図上の心拍数と不整脈を指標として検討した。

　アトロピン使用群では，局所麻酔後に平均脈拍数が25％以上増加した症例が多くみられ，その傾向は70歳以上の高齢者において顕著であった。一方，非使用群では手術後に平均脈拍数が10％以上減少する症例が多くみられた。しかしながら，いずれの群においても，眼心臓反射に由来すると考えられる手術中に突然発生する徐脈はみられなかった。上室性期外収縮は手術過程にはほとんど影響されなかった。一方，心室性期外収縮は，非使用群では前投薬後に一過性に増加し，その後は減少する傾向がみられたが，アトロピン使用群では心室性期外収縮が持続して増加する傾向がみられた。最後に，アトロピンにより誘発された狭心症発作例を供覧した。アトロピンは，ことに高齢者においては循環動態に対する影響が強いため，局所麻酔下眼科手術にルーチンに使用する必要はないものと考えた。

We made a prospective study on the merits andside effects of systemic atropine sulfate aspremedication to ophthalmic surgery under localanesthesia. In the first group, a consecutive seriesof 100 patients received 0.5 mg of atropine sulfateprior to surgery. The heart rate increased by anaverage of 25%, particularly in persons over 70years of age. Ventricular extrasystole also in-creased and persisted for hours. Attack of anginapectoris developed in one patient. In the secondgroup of 100 patients without prior atropine, theheart rate decreased by 10% and ventricularextrasystole increased temporarily after start ofsurgery. We advocate against indiscriminate use ofatropine sulfate prior to ophthalmic surgery, partic-ularly in aged persons.
