片眼白内障に対して多焦点眼内レンズを挿入した若年症例の検討

要約　目的：片眼の白内障に対して多焦点眼内レンズの挿入を受けた若年者の検討。対象と方法：過去23か月間に片眼のみに白内障手術を受けた14例を対象とした。男性13例，女性1例で，全例が50歳以下，平均37.9±8.8歳である。白内障の原因は，アトピー性皮膚炎8例，外傷1例，原因不明5例である。4種類の多焦点眼内レンズを挿入し，1か月後に評価した。結果：遠方視力は無矯正で1.0以上が12例（86％），矯正時で全例が1.0であった。無矯正での近方視力は6例（43％）が0.7以上であった。全例での焦点深度曲線は2峰性で，視力は＋1D～－4D加入の全領域で0.7以上であった。アンケートによる患者満足度は，遠方と近方ともに良好であった。ハローまたはグレアが8例にあったが，軽度であり生活に支障はなかった。僚眼は眼鏡無使用が10例（72％），コンタクトレンズ装用が3例（21％），1例が術後にLASIKを受けた。結論：片眼に多焦点眼内レンズ挿入を受けた50歳以下の成人では，眼鏡なしに遠方と近方の良好な視力が得られた。

Abstract.　Purpose：To report the outcome of unilateral implantation of multifocal intraocular lens（IOL）in young persons. Cases and Methods：This retrospective study was made on 14 persons who received unilateral insertion of multifocal IOL in the past 23 months. The series comprised 13 males and one female. The age was below 49 years,average 37.9±8.8 years. Cataract was due to atopic dermatitis 8 cases,trauma one case,and unknown 5 cases. Four types of IOL were used. Results：Far visual acuity（VA）was 1.0 or over in 11 eyes（79％）and near VA was 0.7 or over in all the eyes without correction. The focal depth showed a bimodal pattern. Uncorrected VA was 0.7 or over in all the range from ＋1 to －4 diopters. All the patients expressed satisfaction regarding near and far VA. Halo or glare was present in 8 eyes but was insignificant in daily life. Conclusion：Persons under 49 years of age were satisfied regarding near and far VA without correction after unilateral implantation of multifocal IOL.
