小瞳孔眼への眼内レンズ移植術

Extracapsular cataract extraction and intraocular lens implantation in eyes with small pupil

　虹彩後癒着を伴う小瞳孔の白内障眼11眼に，虹彩全幅切開と再縫合を含む計面的嚢外白内障手術および眼内レンズ移植術を行った。重篤な術中術後の併発症はなく，視力予後は良好であった。症例の大多数は，虹彩切開術などの処置後の特発性閉塞隅角緑内障眼であったが，術前よりも眼圧コントロールが容易となった。術前後の隅角所見などからその理由は，前方移動した水晶体による虹彩根部の圧迫と，不完全に残存する瞳孔ブロックが解除されたためと推測した。これらの症例に対する眼内レンズ手術は比較的容易で，重篤な併発症も少なく，視力の改善効果と緑内障治療を目的として積極的に試みてよい。

We performed extracapsular cataract surgery and posterior chamber intraocular lens implanta-tion in 11 eyes with fixed small pupil. The small pupil was secondary to treated primary angle clo-sure glaucoma in 10 eyes and to healed uveitis in 1. Surgery was facilited by iridoplasty and envelope technique. No complications of note developedduring or after surgery. Visual acuity improved to 20/40 or more in 9 eyes. The chamber angle became open except occasional peripheral synechiae. In all the 10 eyes with glaucoma, the intraocular pressure was controlled below 20 mmHg, including 4 out of 7 eyes which required no or lesser medication after surgery.
