穿孔性眼外傷における予後不良例の検討

Characteristics of penetrating ocular injury with poor visual outcome

　1989年11月より1990年8月までの22か月間に，多根記念眼科病院で処置をした穿孔性眼外傷28例30眼について，視力予後不良の要因を検討した。23眼（76.6％）の症例で最終矯正視力0.7以上を得たが，5眼（16.7％）が0.3以下であった。重傷例5眼のうち4眼が網膜剥離を併発しており，予後に与える最も大きな因子であった。外力が大きいほど，それによる損傷も大きく予後不良となったが，さほど大きくなくても，黄斑部を直接損傷すれば，視力予後は不良であった。前眼部（四直筋付着部より前方）の穿孔は透光体の混濁を残さなければ予後良好であったが，毛様体動脈を損傷し持続性出血をきたし，予後不良となった例があった。

We evaluated 30 consecutive cases of penetrating ocular injury regarding visual outcome seen by us during a 10-month-period. Final visual acuity of 0.7 or more was attained in 23 eyes, 77%. Visual acuity of 0.3 or less resulted in 5 eyes, 17%. Retinal detach-ment developed in 4 of these 5 eyes. Poor visual acuity was due to macular pucker in one eye, ocular hypotension with silicone tamponade in one, and persistent retinal detachment in 2. Poor visual acuity resulted in another case of anterior segment injury with massive hemorrhage.
