悪性リンパ腫によるぶどう膜炎の1例

A case of intraocular malignant lymphoma

　悪性リンパ腫によるぶどう膜炎の1例を経験した。症例は38歳，女性。初診時視力は右1.5，左0.08であった。左眼後極部には大型でやや隆起した黄白色病巣，黄斑部を含んだ漿液性網膜剥離とその周囲に灰白色の小病巣が散在してみられ，鼻側周辺部には黄白色の隆起病巣が集簇していた。頭部CTに異常はみられなかったが，眼・中枢神経系原発悪性リンパ腫に相当する眼内悪性リンパ腫ではないかと考え，診断的硝子体手術を施行し，硝子体中に異型細胞が認められた。放射線照射（総量50Gy）を行い，眼底所見の著明な改善がみられた。本症は，早期に詳細な眼底病変の検索が行えれば，臨床的に診断可能ではないかと考えられたが，確定診断としては，診断的硝子体切除術を応用するのが安全ではないかと考えられた。

A 38-year-old female presented with progressive visual impairment in her left eye. The affected eye showed yellowish and dome-shaped lesion in the posterior fundus with localized retinal detachment. Fluorescein angiography showed blocked fluores-cence in the elevated area, suggesting the causativelesion to be located anterior to the choroid. The clinical findings were suggestive of intraocular malignant lymphoma. Diagnostic vitrectomy showed the vitreous to contain class 3 atypical cells. Systemic corticosteroid proved futile. The infiltrative chorioretinal lesions responded well to radiation therapy totalling 45Gy.
