螢光眼底像の画像処理．おもにドルーゼンについて

Image processing of fluorescein angiogram of age-related drusen

　加齢黄斑変性症症例69例の螢光眼底像の画像強調を行い，特にドルーゼンについて検討し，以下のような結論を得た。①硬性ドルーゼンは，著明に強調され，オリジナル像で不鮮明なものも明瞭に描出された。②ごく小型の硬性ドルーゼンと思われる点状螢光や顆粒状の螢光が広い範囲にみられ，広範囲の網膜色素上皮の障害が示唆される症例があった。③大型の軟性ドルーゼンの内部に点状螢光が混在している例があった。④大型の軟性ドルーゼンで，不規則な形で濃淡のある過螢光を示す例があり，ドルーゼンの退縮，融解の始まりの所見ではないかと考えられた。螢光眼底像の画像処理は，ドルーゼンの臨床分類を考えるうえで有用であると考えられた。

We applied computerized image processing to fluorescein angiograms of age-related macular degeneration (ARMD) to achieve contrast enhance-ment. The series comprised 69 eyes with drusen: 44 with neovascular type ARMD and 25 with ARMD without choroidal neovascularization.

Enhance effect was more pronounced in hard drusen. The processing made smaller drusen observ-able. Image quality of large-sized soft drusen im-proved by far lesser degree. In some eyes, image enhancement showed numerous spotty or granular hyperfluorescences over a wide fundus area, sugges-ting the presence of diffuse lesions in the retinal pigment epithelium. Some large-sized soft drusen showed to contain hyperfluorescent dots by enhan-cement. Our image-processing procedure were thus useful in detailed clinical classification of age -related drusen
