A 55-year-old male underwent secondary im-plantation of posterior chamber lens 3 years after extracapsular cataract extraction. Five years later, he presented with bullous keratopathy in the treat-ed eye. The posterior chamber lens was tilted and one end of the haptic loops was dislocated in the anterior chamber through an iris cleft at one o'clock position and was in apposition to the cor-neal endothelium. A position-correction maneuver induced complete resolution of bullous keratopathy leaving marked megarhythm of the corneal endoth-elium. Due precaution is necessary to properly fixate the lens in attempting secondary posterior chamber implantation in eyes with previous extracapsular cataract extraction and peripheral iridectomy, particularly in those with posterior capsular rupture.
