A total of 6,740 cataract surgeries were performed in our institution during the foregoing 10 years. A second surgery became necessary in 17 eyes to repostion, remove or replace the intraocular lens (IOL) for intra-or postoperative complications. The series comprised 7 eyes of dislocated IOL, 5 eyes of prolapse of IOL into the anterior chamber or the vitreous, 3 eyes of postoperative endophthalmitis and 2 eyes of miscalculated refractive power. The majority of the 17 eyes manifested some intraoperative problems. Meticulous care during the initial cataract surgery thus appeared to be important in avoiding later complica-tions. A second surgery became necessary more than one year after the initial cataract surgery in 7 of the 17 eyes, 41%. A longterm monitoring is advocated for cases with poor or asymmetrical IOL fixation.
