ミトコンドリアサイトパチーの1例

A case of mitochondrial cytopathy with ophthalmoplegia

　今回筆者らは，眼瞼下垂，複視を主訴とする慢性進行性外眼筋麻痺の16歳の女性について，外斜視手術時に得られた外眼筋組織検査および大腿直筋の筋生検を行い，組織学的にミトコンドリアサイトパチーの確診を得るに至った。本症例では心伝導障害，網膜変性はみられなかったが，両上肢の近位筋の筋力低下が認められた。手術で得られた内直筋と大腿直筋にミトコンドリアの異常がみられ，大腿直筋のGomori-trichrome染色でragged-red fiberを認めたことより，本症の確定診断を得た。また電子顕微鏡の所見では，変性の強い部位で筋線維はほとんど消失し，膨化，または変性したミトコンドリアを多数認め，これらの所見は内直筋に著明であった。

A 16-year-old girl presented with bilateral ble-pharoptosis and exotropia with diplopia since 6 years before. There were no abnormalities in the fundus and the ocular adnexa. Physical examina-tions disclosed weakness in the arm muscles. Biop-sy showed ragged-red fibers in the femoral rectusmuscle by Gomori trichrome staining. The medial rectus muscle in the right eye showed accumulation of degenerated or swollen mitochondria and decrease of muscle fibers by electron microscopy. These findings led to the diagnosis of mitochon-drial cytopathy. Cosmetic improvement was attained by resection of right medial rectus muscle and recession of left lateral rectus muscle.
