角膜角化症と考えられる1例

A case of presumed corneal keratinization

　角膜角化症と考えられる1例を報告した。症例は61歳の女性で，左眼視力低下と異物感を主訴とした。前眼部所見では，左眼下側角膜輪部から中央部に向かって棒状の膜様物が侵入していた。治療的生検の目的で切除した標本を組織学的に検討した。光顕像では，重層した扁平上皮細胞がみられ，表層の細胞は核を失い，角化していた。電顕像で，表層部に無核，無構造な角化傾向を示す細胞が存在し，基底部においては細胞質内細線維が発達しており，角化像が示された。隣接細胞間は多数のデスモゾームで連結していた。なお，膜様物切除後，左眼視力は（1.0）に改善し，現時点において膜様物の再発は経験していない。

A 61-year-old female presented with impaired vision and foreign body sensation of one month's duration. Her left cornea was covered by a mem-brane extending over the inferior sector. We perfor-med therapeutic biopsy. Light microscopy showed stratified squamous cells. The superficial cells hadundergone keratinization without nucleus. By elec-tron microscopy, the superficial layers consisted of keratinized cells without nucleus. The deeper layers were characterized by accumulation of tonofila-ments in the cytoplasm and by dyskeratosis. There were numerous intercellular desmosomes. These findings led us to the diagnosis of corneal keratin-ization. The therapeutic biopsy was followed by eventual cure and improvement in visual acuity.
