前房内に自然排出した眼内異物の症例

A case of spontaneous emigration of intraocular foreign body into the anterior chamber

　臨床所見より眼球鉄症を疑いながら，画像診断では異物の確認ができず，経過観察中に前房内に異物が自然排出した1症例を経験した。症例は36歳男性。角膜に穿孔創はなく，水晶体は全体に混濁し，前嚢下にrust spotがあった。ERGではb波低下，律動様小波の消失があった。また摘出水晶体は鉄染色陽性であった。その後，隅角下方に異物が自然排出した。摘出した異物の主成分は樹脂で，ごく微量の鉄を含有していたため，非金属に含有された鉄により眼球鉄症を発症したと結論した。画像診断上異物が確認できない症例でも，非金属に含まれる微量の鉄により，眼球鉄症が発症する可能性が示唆された。

A 36-year-old male noticed progressive blurring in his left eye during the past 2 years. We observed dense cataract and pigment deposit, or rust spot, on the anterior lens surfce. Signs of penetrating ocular injury were absent by observations with slitlamp biomicroscope, x-ray or computed tomography. We performed cataract extraction. The affectedlens showed positive iron reaction histologically. He was diagnosed as ocular siderosis. An intraocular foreign body appeared spontaneously in the anterior chamber 2 years later. Radiomi-croanalysis of the removed foreign body showed it to be composed of resin containing a small amount of iron. The present case illustrates that ocular siderosis may be induced by an apparently non -ferrous material containing an iron component.
