Terrien's marginal degenerationに対する表層角膜移植

Lamellar keratoplasty for marginal degeneration of the cornea (Terrien)

　Terrien's marginal degenerationの3症例に対し，いささかの工夫を凝らし表層角膜移植術を行った．術後全症例で透明治癒し，また角膜の変形も矯正でき，視力の向上を認め，術後各1年，7年8カ月，2年2カ月経ているが，再発もおこらなかった．術前，術後にわたりphotokera-toscopeを用い角膜の形状をとらえ，術前形状変化をきたしていた角膜も，表層角膜移植術の結果，角膜の変形が矯正されていた．また，Terrien'smarginal degenerationに対する全層あるいは表層（全表層または周辺部）角膜移植術の選択，手術時期について考察を加えた．

We treated 3 cases (3 eyes) with Terrien's marginal degeneration of the cornea by lamellar keratoplasty to relieve corneal astigmatism. The patients were aged 19, 36, 49 years each at the time of surgery.

The treatment was a success as the visual acuity improved significantly after surgery. The grafted cor-nea was clear and was free of recurrences for at least one year after surgery. Evaluation of corneal configura-tion by photokeratoscope showed that the corneal deformity was favorably corrected by surgery.

Lamellar keratoplasty is recommended as the treat-ment of choice to improve vision in Terrien's marginal degeneration of the cornea.
