脂腺癌と診断された眼窩腫瘍

A case of sebaceous carcinoma of the orbit

　脂腺癌と診断された眼窩腫瘍の1例を報告する。症例は60歳，女性。涙丘部に進展する眼窩腫瘍を認め，眼球は上耳側に偏位していた。腫瘍を摘出し，放射線照射を行った。術後2年6か月後の現在まで再発や全身転移はない。病理学的検査では，腫瘍は胞巣状構造をとり，腫瘍細胞はやや大型で，明らかな異形性を認め，胞体は大小の空胞を有していた。脂肪染色陽性であり，脂腺癌と診断した。眼窩以外に腫瘍性病変を認めず，涙丘原発と考えられた。脂腺癌が涙丘より発生しうること，臨床的にはしばしば眼窩腫瘍として認められること，診断には脂肪染色が必須であることを強調したい。

A 60-year-old female presented with orbital tumor in her left eye of 27 months' duration. The eyeglobe was displaced superotemporally. Comput-ed tomography and magnetic resonance imaging showed a homogenous tumor mass nasal inferior tothe eyeglobe. The tumor was extirpated through transseptal approach followed by radiation total-ling 50 Gy. Histological studies showed prolifera-tion of large tumor cells with metamorphism arran-ged in a lobulated structure. Presence of foamy or vacuolated cytoplasm and positive fat stain led to the diagnosis of sebaceous carcinoma originating from the lacrimal caruncle. There has been no local recurrence nor metastasis during 30 months after surgery.
