病理診断が困難であった眼窩悪性リンパ腫の1例

A case of orbital lymphoma with atypical histopathological features

　病理組織学的に診断が困難であった眼窩悪性リンパ腫の1例を報告した。症例は76歳，男性で，眼球突出（左）を指摘され，当科を紹介された。左眼の上転障害があり，CTにて球後にびまん性に広がる腫瘍を認めた。クレーンライン手術により腫瘍を亜全摘し，放射線照射を行った。1年後の現在までのところ再発はない。

　病理組織像は小型リンパ球を主体とするリンパ組織の増殖を示し，多数の形質細胞が認められた。異型性は明らかでなかったが，核に軽度のくびれがみられる細胞がみられ，免疫組織化学的にκ鎖，IgMが陽性で単クローン性が証明され，悪性リンパ腫（lymphoplasmacytic type）と診断した。

A 76-year-old male was referred to us because of left exophthalmos. Supraduction was restricted. Computed tomography showed a diffuse mass lesion in the retrobulbar space. He was treated by subtotal resection through Kroenlein's approach followed by radiation therapy. No recurrence or systemic involvement during one year after sur-gery.

Histopathological studies revealed proliferation of small lymphocytes with marked infiltration of plasma cells. There was no apparent metamor-phism except slight notching of the nucleus in a certain number of cells. Demonstration of apparent monoclonality with positive kappa chain and IgM by immunohistochemical technique led to the di-agnosis of malignant lymphoma of lymphoplas-macytic type.
