眼窩リンパ球系細胞増殖性病変におけるフローサイトメトリーの応用

Evaluation of orbital lymphoid tumors by flowcytometry

眼窩偽腫瘍4例，木村病1例，悪性リンパ腫6例においてフローサイトメトリーにより腫瘍を構成するリンパ球表面マーカーを検討した。病理学的，免疫組織学的に診断が確実なもののみを対象とした。悪性リンパ腫はすべてびまん性リンパ腫で，B細胞性であった。リンパ過形成ではCD3陽性細胞は44.3±10.0％，CD20陽性細胞は53.2±14.7％で，悪性リンパ腫ではCD3陽性細胞は24.8±10.7％，CD20陽性細胞は78.6±17.7％で，悪性リンパ腫ではCD20陽性細胞が優位であった。κ／λ比では，リンパ過形成では2.03±1.32，悪性リンパ腫では34.62±19.92で，腫瘍組織のフローサイトメトリーによる検索にて単クローン性は容易に判定可能であった。（数値は平均±標準偏差を示す）

We performed flowcytometric analysis of lymphocyte surface markers in 11 eyes of orbital tumors. The series included orbital pseudotumor 4 eyes, Kimura's disease 1 eye and malignant lympho-ma of the orbit 6 eyes. All the cases were eventually diagnosed as malignant lymphoma of diffuse and B lymphoma on histopathological and immunohis-topathological grounds. In eyes with lymphoid hyperplasia, CD3-positive cells were 44.3±10.0%(mean±s. d.) and CD20-positive ones were 53.2±14.7%. In eyes with malignant lymphoma, CD3 -positive cells were 24.2±10.7% and CD20-posi-tive one were 78.6±17.7%. CD20-positive cells were thus markedly predominant in malignant lymphoma. x/λ ratio was 2.0±1.3 in lymphoid hyperplasia and 34.6±19.9 in malignant lymphoma. Monoclonal proliferation was readily diagnosed by flowcyteometric analysis of the tumor tissue.
