アルカリホスファターゼ活性が上昇した原発性結膜下悪性リンパ腫の1例

A case of primary subconjunctival malignant lymphoma with elevated alkaline phosphatase activity

　結膜に原発することは稀である，悪性リンパ腫の1例を経験した。症例は43歳の男性で主訴は左眼の眼瞼下垂と霧視であった。左眼上方球結膜にみられたサーモンピンクの色調の腫瘍は，切除術後の病理組織診断の結果，非ホジキン型リンパ腫であり，LSG分類のびまん性リンパ腫・中細胞型に該当した。免疫組織化学的に腫瘍細胞は，B細胞型で，細胞膜に認められた免疫グロブリンは，IgMχ陽性であった。血清免疫グロブリン中，IgMに軽度上昇がみられたが，全身リンパ節に腫脹はなく，全身検査上も異常を認めず，Ann-Arbor分類Ⅰ期であった。腫瘍切除術後に放射線療法を行い経過は良好である。

A 43-year-old male presented with a subconjun-ctival pinkish mass in the left eye. Histologically, the tumor was diagnosed as diffuse malignantlymphoma of medium-sized cell type. Immunohis-tochemically, the tumor cells were positive for MB -1 and L-26. They showed intense alkaline phos-phatase activity indicating B-cell lymphoma. The immunoglobulin in the cell membrane was positive for IgMχ. Systemic and laboratory findings were negative except for slight increase of IgM.
