We performed extracapsular cataract surgery in 30 eyes using intercapsular technique and posterior chamber lens implantation. The lens epithelium was removed by phacoemulsification. Another series of 13 eyes were treated by the same surgical technique without intraocualr lens (IOL) implanta-tion. The follow-up averaged 30 and 15 months each. Fibrous opacification of the anterior capsule developed 25 pseudophakic eyes, 83%. The opacification developed at the site of contact with the IOL. The region beyond the margin of IOLremained transparent. No fibrous opacification developed in the 13 aphakic eyes. This finding seemed to suggest that polymethyl metacrylate, PMMA, stimulated the lens epithelial cells to undergo fibrous metaplasia and to produce col-lagen. Interleukin-1, which is synthesized by lens epithelial cells, may serve as a mediator. This feature will have to be considered regarding biocompatibility in the development and evaluation of new biomaterials.
