A 55-year-old male underwent uneventful cataract surgery with in-the-bag implantation of 11mm plano-convex intraocular lens (IOL). Spontaneous total posterior dislocation of IOL with the lenscapsule occurred 32 months later. The eye was treated by vitrectomy and the IOL was removed by hookingthe opening of fibrotic capsule with a bent needle. A posterior chamber lens with modified C-loops wasinserted and sutured to the ciliary sulcus. Histological study of removed IOL showed that the migration oflens epithelial cells had stopped at the optic edge. The present surgical technique promises to be of value inthe management of posteriorly dislocated IOL.
