原田病の髄液所見と眼症状

Cerebrospinal fluid and ocular manifestations in Vogt-Koyanagi-Harada disease

　原田病患者30例について房水と髄液の蛋白濃度と細胞数を比較検討した。このうち房水蛋白濃度を新鮮例11例と遷延例2例に対し，レーザーフレアセルメーターで測定した。髄液蛋白濃度が正常域（43mg／dl未満）にとどまる新鮮例の平均房水蛋白濃度は36.7mg／dlであるのに対し，髄液蛋白濃度が正常域を越える症例の平均房水蛋白濃度は65.4 mg／dlであり有意に高値を示した。髄液蛋白濃度はさらに房水細胞数とも相関した。新鮮例の髄液蛋白濃度と髄液細胞数は，予後に関して，治癒と遷延化した症例の間で差がなかった。遷延例の髄液蛋白濃度と細胞数は正常範囲にあった。発病初期原田病の髄液蛋白濃度はその時点での前眼部炎症と相関すると判断された。

We quantitated the protein concentration and cells in the cerebrospinal fluid and the aqueous humor in 30 patients with Vogt-Koyanagi-Harada disease. The disease was in its active stage in 28 and in protracted one in 2. In patients during the acute stage of the disease, or within 57 days afteronset, protein concentration in the cerebrospinal fluid was positively correlated with protein concen-tration and cells in the aqueous (p＜0.01). Protein in the cerebrospinal fluid did not correlate with the ocular prognosis. In protracted cases with active iritis, cerebrospinal fluid showed normal levels of protein and cells.
