名古屋市立大学病院におけるサルコイドーシスの統計的観察（第2報）

Statistical study of sarcoidosis in Nagoya City University Hospital. Report 2.

　過去7年間に，名古屋市立大学病院眼科でサルコイドーシス（以下，サ症）を疑った58例（Ⅰ群），同院他科でサ症と診断され眼科を受診した55例（Ⅱ群）の初診時以降の眼病変の経過，治療および視力予後などを，6か月以上経過観察できた症例について検討した。Ⅰ群で25例，Ⅱ群で13例に新たな眼病変が出現した。出現した病変数は1個が多く，時期は80％以上が初診時から1年以内であり，種類は網膜血管周囲炎が多くみられた。Ⅰ群は眼科的に重篤であり，多数の症例でステロイド剤の局所注射や内服治療を必要とし，眼合併症，視力予後の不良例も多かった。初診時から最低1年間は厳重な眼科的管理が必要である。

We reviewed 58 cases of presumed monosympto-matic ocular sarcoidosis (group 1) and 55 cases of sarcoidosis with ocular and systemic involvements (group 2) during the foregoing 7 years. During the follow-up period, 25 cases in group 1 and 13 cases in group 2 manifested other typical signs of sar-coidosis which were absent initially. Retinal periph-lebitis was the most frequent one. These signs became manifest within one year in over 80% of the cases. Cases in group 1 tended to show severer ocular manifestations than in group 2, requiring topical and systemic corticosteroid therapy. Secon-dary glaucoma, complicated cataract and cystoid macular edema were also more frequent in the former. We advocate close follow-up of cases of sarcoidosis for at least one year after detection.
