Abstract Purpose:To evaluate the relationship between subjective refraction, astigmatism and uncorrected visual acuity(UCVA)in monofocal pseudophakic eyes.

Cases and Method:This study included 198 eyes of 102 consecutive patients who underwent monofocal IOL implantation during cataract surgery, and who had a best-corrected visual acuity≥1.0 at postoperative 1 week. The mean age of the subjects was 71.4±11.3 years and the mean postoperative subjective refraction was −1.01±1.19D(spherical equivalent). We evaluated the correlation between postoperative refraction and the uncorrected distance visual acuity(5 m, UDVA)and uncorrected near visual acuity(33 cm, UNVA). Visual acuity was evaluated as logMAR.

Results:Variations were observed between the UDVA and UNVA at each subjective refraction. A significant correlation was observed between subjective refraction(x)and UCVA(y). The linear relationship between the two could be approximated by the equation y=−0.286x+0.04 for UDVA and y=0.199x+0.643 for UNVA. From these formulas, it is possible to estimate postoperative UDVA and UNVA. The presence of astigmatism showed a significant difference in UDVA(p<0.001), but there was no significant difference in UNVA.

Conclusion:When determining IOL power, it is important to understand the UCVA distribution in relation to subjective refraction, and to share this information with the patient. It is necessary to examine the necessity of corrective astigmatism and corrective method by target refraction.
