Abstract Purpose:Chemotherapeutic drugs may cause ocular complications such as keratoconjunctival disorders, lacrimal passage disorders and retinal disorders. Cystoid macular edema is a rare adverse effect of taxanes, including paclitaxel. Herein, we report a case of bilateral cystoid macular edema that was associated with paclitaxel therapy for breast cancer. Improvement was noted after the cessation of paclitaxel treatment.

Case:A 61-year-old woman with breast cancer and liver metastasis was undergoing treatment with paclitaxel and bevacizumab. She experienced deterioration of vision in both eyes five months after starting paclitaxel therapy, prompting a consultation at Toyohashi Municipal Hospital ophthalmology department.

Findings:Her best-corrected visual acuity was 0.6 in the right eye and 0.5 in the left eye. Anterior segmental examination revealed normal findings and optical coherence tomography revealed bilateral cystoid macular edema. Fluorescein angiography did not reveal any abnormality. One and a half months after discontinuing paclitaxel, the patient's best-corrected visual acuity improved to 1.5 in both eyes and optical coherence tomography revealed resolution of the edema.

Conclusion:The patient's cystoid macular edema improved following paclitaxel discontinuation. Optical coherence tomography and fluorescein angiography are extremely useful tests for the early detection of cystoid macular edema.
