要約　目的：白内障手術後，トラボプロストを点眼中に囊胞様黄斑浮腫が生じた症例の報告。症例：70歳女性が視力低下で受診した。所見：矯正視力は右0.5，左1.0で，右眼に皮質白内障があり，眼圧は右27mmHg，左26mmHgで，高眼圧症と診断した。タフルプロストを1日1回点眼し，2か月後に右眼に白内障手術を強角膜切開で行い，アクリルレンズを挿入した。手術の2か月後にタフルプロスト点眼を再開し，3週間後にトラボプロスト点眼に切り替えた。4週間後に視力が0.5に低下し，光干渉断層計（OCT）で囊胞様黄斑浮腫があった。トラボプロストを中止し，視力はすみやかに改善し，囊胞様黄斑浮腫は消失した。結論：プロスタグランジン製剤による白内障術後の囊胞様黄斑浮腫は，主剤よりも防腐剤による可能性があるが，トラボプロスト点眼でも発症しうる。

Abstract.　Purpose：To report a case who developed cystoid macular edema（CME）during instillation of travoprost after cataract surgery. Case：A 70-year-old female presented with impaired visual acuity in her right eye. Findings：Corrected visual acuity was 0.5 right and 1.0 left. The right eye showed cortical cataract. Intraocular pressure（IOP）was 27 mmHg right and 26 mmHg left. She started topical instillation of tafluprost ophthalmic solution. She received cataract surgery with insertion of acryl intraocular lens in the right eye. After 8 weeks tafluprost was restarted and shifted to travoprost 3 weeks later. Visual acuity decreased to 0.5 4 weeks later. Optical coherence tomography（OCT）showed CME. Discontinuation of travoprost was followed by disappearance of CME with increased visual acuity. Conclusion：This case illustrates that CME may be induced by instillation of preservative-free travoprost.
