Abstract Purpose:Toxic anterior segment syndrome(TASS)is said to be an inflammatory disease that develops in the anterior segment due to non-infectious substances after intraocular surgery such as cataract surgery. We report two consecutive cases suspected of subacute-onset TASS after insertion of intraocular lens.

Case:A 73-year-old woman and a 71-year-old man underwent cataract surgery at Showa University East Hospital. There were no intraoperative complications. The day after surgery, Descemet fold was mildly observed in one case. Inflammation in the anterior chamber was mild in both cases. Corrected visual acuity improved to 1.0 and 1.2 respectively. On 12th day of surgery, both patients were aware of blurred vision. They showed ciliary injection, painless anterior chamber inflammation with fibrin deposition in the pupillary area and posterior synechia. One case showed the descemet fold. Corrected visual acuity decreased to 0.4 in both cases. Increase in flare value was observed with a laser flare cell meter. From these results, we suspected late-onset TASS. From the 12th day of surgery, in addition to antibacterials and corticosteroids, nonsteroidal anti-inflammatory drugs and mydriatics were added. In both cases, inflammation in the anterior chamber and descemet fold gradually improved only by eye-drop. Although one posterior synechia remained 6 months after surgery, corrected visual acuity improved to 1.2 in both cases. Subjective symptoms improved respectively.

Conclusion:These cases illustrate that surgery may cause late-onset TASS.
