眼内レンズ挿入術後に発生した表皮性ぶどう球菌による慢性細菌性眼内炎の2例

Two cases of chronic endophthalmitis due to Staphylococcus epidermidis after intraocular lens implantation

　水晶体嚢外摘出術および眼内レンズ挿入術（ECCE＋IOL）後に表皮性ぶどう球菌による慢性細菌性眼内炎が65歳と62歳の女性に発症した。2例とも臨床症状が軽度であったため術後の無菌性眼内炎としてステロイドの局所投与を主体に治療を行っていたが，治療に反応しないため術後50日過ぎに前房水培養を行ったところ，表皮性ぶどう球菌が同定された。1例は抗生剤の点眼と結膜下注射のみで治癒したが，1例は硝子体手術を要した。

　ECCE＋IOL後の前房内炎症が遷延する症例には，慢性細菌性眼内炎の可能性を考え，積極的に前房水培養を検討すべきと考えられた。

We observed chronic endophthalmitis in 2 females, aged 62 and 65 years, following uneventful extracapsular cataract extraction with posterior chamber intraocular lens implantation (PCIOL). Both cases were characterized by mild but pro-longed inflammatory signs and by poor response to topical corticosteroid. Culture of anterior aqueous led to the identification of Staphylococcus epider-midis 50 days after surgery. Topical and systemic antibiotics led to cure in one case. Additional pars plana vitrectomy was necessary in the other.
