A research engineer suffered from an accidental burn of the macula in his left eye by Nd: YAG laser. He was 43 years of age and had been engaged in research works with lasers for the past 20 years. He was not wearing protective goggles. When the laser hit his left eye, he was experimenting with Q -switched Nd: YAG laser with the wavelength of 1,063 μgm, power output of 100 mJ, pulse duration of 6 ns, repetition rate of 20 Hz and beam diameter of6 mm. Blurred vision and myodesopsia developed immediately after the accident.

When seen 2 days later, the visual acuity was 0.1 with best correction in the left eye. A macular hole had formed about one-third disc diameter in size. The hole was surrounded by retinal edema and hemorrhagic patch. Diffuse vitreous hemorrhage was present anterior to the macula.

The hemorrhages and edema disappeared 10 days after the injury, resulting in final visual acuity of 0.1 with a paracentral scotoma of 2° in size.
