Abstract Purpose:To report a case of intraocular iron foreign body removed one year after injury.

Case:A 14-year-old boy presented with mature cataract and a full-thickness corneal scar in the right eye. He told that something flew into the right eye while hitting two hammers one year ago.

Findings and Clinical Course:Visual acuity was hand motion in the right eye. Ultrasonography showed retinal detachment and high-density material. Computed tomography showed the presence of iron foreign showed body. Electroretinogram(ERG)showed reduced b-wave suggesting ocular siderosis. The right eye received phacoemulsification with aspiration, vitrectomy, scleral encircling and silicone oil tamponade for proliferative vitreoretinopathy with subretinal proliferation. A foreign body was removed. Silicone oil was removed 3 month later followed by intraocular lens implantation. Visual acuity improved to 1.2.

Conclusion:This case illustrates that ERG is useful in evaluating the visual function of ocular siderosis, but that its amplitude and latency may be influenced by the presence of vitreoretinopathy.
