老視用角膜インレー挿入眼に対する硝子体手術

Vitreous surgery in eyes implanted with corneal inlay for presbyopia

要約　目的：老視に対する角膜インレー挿入眼に裂孔原性網膜剝離を発症し，硝子体手術を施行した2症例を報告する。

症例：症例は61歳女性および66歳女性，両者とも2012年に両眼に角膜屈折矯正手術，片眼に老視用角膜インレー（KAMRA，AcuFocus社）挿入術を施行されている。それぞれ2および3年後，インレー挿入眼に裂孔原性網膜剝離を発症した。2症例とも角膜インレーを温存した25ゲージ硝子体手術を施行し，網膜の復位を得た。

結論：角膜インレー挿入眼ではインレーを避けて観察する必要があり，広角観察システムが有用であった。インレー挿入眼の内眼手術では，待機的な治療であればインレーの除去も検討すべきである。

Abstract　Purpose: To report vitreous surgery for retinal detachment in 2 eyes implanted with corneal inlay for presbyopia.

Cases: Rhegmatogenous retinal detachment developed in 2 females aged 61 and 66 years respectively. Corneal inlay had been implanted 2 and 3 years before respectively. Both cases received 25-gauge 3-port pars plana vitrectomy without removal of corneal inlay. The retina became reattached after surgery.

Conclusion: The dark area of corneal inlay may disturb visibility of the fundus during vitreous surgery. The wide-angle viewing system provided clear view of the fundus in eyes implanted with corneal inlay.
