Abstract Purpose:To report a case of acute retinal necrosis who showed atypical findings.

Case:A 69-year-old female presented with one-month history of decreased vision in her right eye.

Findings:Corrected visual acuity was 0.06 right and 0.6 left. The right eye showed anterior uveitis, vitreous opacity, occlusive vasculitis, and optic atrophy. Varicella zoster virus was detected from and aqueous humor by PCR. Two weeks later, the right eye developed total retinal detachment. She was treated by systemic acyclovir, vitrectomy and silicone oil tamponade.

Conclusion:The present case showed relatively mild inflammatory findings of the involved eye. This case illustrates that clinical features of necrotizing herpetic retinopathies may be determined by the immune status of the host.
