Abstract Purpose:To report an elderly case of unilateral anterior ischemic optic neuropathy(AION)with positive anti-neutrophil cytoplasmic antibody(ANCA)for myeloperoxidase.

Case:A 80-year-old male was referred to us for suspected AION. He had had dull pain in the head, neck and waist since one month before. He had reportedly impaired vision in the left eye and horizontal hemianopia.

Findings and Clinical Course:Visual acuity was 1.5 right and 0.1 left. The left eye showed positive RAPD and hyperemia with swelling of optic disc. Hematological findings were suggestive of AION with giant cell arteritis. The patient was finally diagnosed with AION with MPO-ANCA-related vasculitis. No other organs showed symptoms related with vasculitis. After treatment with systemic corticosteroid, the left visual acuity improved to 0.4 with no changes in subjective findings. There was no recurrence of AION after reduction of corticosteroid.

Conclusion:This case illustrates that MPO-ANCA vasculitis may simulate giant cell arteritis as the underlying cause of AION.
