要約　目的：角膜内皮細胞障害を伴った難治性緑内障に対し，バルベルト緑内障インプラント（BGI）のチューブ先端を毛様溝から挿入した症例の報告。症例：62歳，男性。25年前に右眼の原発開放隅角緑内障と診断。線維柱帯切除術を数回施行後，通院中だったが再び眼圧がコントロール不良になった。結膜の広範な瘢痕のためBGIの適応としたが，角膜内皮細胞障害が判明。毛様体扁平部挿入型インプラントも検討したが，インフォームド・コンセントを得たうえで，前房型インプラントのチューブ先端を毛様溝から後房へ挿入した。結果：術後6か月の現在まで経過良好である。結論：角膜内皮細胞障害を伴った難治性緑内障に対し，当術式は安全で有効であった。

Abstract.　Purpose：To report a case of refractory glaucoma treated by insertion of silicone tube of Baervelt^® implant into the posterior chamber. Case：A 62-year-old male had been diagnosed with primary open-angle glaucoma at the age of 37 years. He had received several sessions of trabeculectomy. His right eye needed another surgical intervention for poor control of intraocular pressure but showed impaired corneal endothelium at 865 cells/mm². After due considerations, a Baervelt^® glaucoma implant was inserted into the posterior chamber through the ciliary sulcus. The right eye has been doing well for 6 months until present. Conclusion：Insertion of Baervelt^® implant into the posterior chamber through the ciliary sulcus was safe and effective in a phakic eye with refractory glaucoma.
