角膜移植後に眼球破裂を生じた9眼の検討

Nine eyes of eyeglobe rupture after corneal transplantation

要約　目的：角膜移植後に眼球破裂を生じた9例9眼の臨床的背景と予後の調査。

対象と方法：2009年4月〜2014年9月に角膜移植後の眼球破裂に対して，東京医科大学病院眼科で加療を行った9例9眼（男性3例，女性6例）の臨床的背景や予後について診療録をもとに調査を行った。

結果：眼球破裂の原因は全例が眼球打撲で，破裂に伴う合併症は眼内レンズの脱出4眼，水晶体の脱出1眼，上脈絡膜出血5眼であった。上脈絡膜出血を合併した症例は最終的に全例が眼球癆に至った。

結論：角膜移植後は長期間経過した後も鈍的外傷による創離開を生じるリスクが高く，予後も不良なことが多い。高齢者など，転倒リスクを有する症例では眼球破裂のリスクについて長期にわたる注意喚起を行い，眼球保護用の眼鏡装用などを勧める必要がある。

Abstract　Purpose: To report the background and outcome of 9 cases who developed eyeglobe rupture following corneal transplantation.

Cases: We reviewed 9 eyes of 9 cases who were seen by us for eyeglobe rupture following corneal transplantation in the past 66 months. The series comprised 3 males and 6 females. The age averaged 72 years.

Findings: Eyeglobe rupture was caused by trauma to the eyeball in all the cases. Eight cases had received penetrating keratoplasty. One case had received deep anterior lamellar keratoplasty. As complications, the lens prolapsed in one case, intraocular lens prolapsed in 4 cases, and suprachoroidal hemorrhage developed in 5 cases. All the eyes of suprachoroidal hemorrhage resulted in phthisis bulbi.

Conclusion: These cases illustrate that eyes that received corneal transplantation have a high risk of eyeglobe rupture due to blunt trauma.
