両側視神経圧迫を起こしたfibrous dysplasiaの1例

A case of fibrous dysplasia with bilateral involvement of the optic canal

　視神経圧迫を起こすfibrous dysplasiaに対し視神経管開放術を施行した症例は数少なく，またその手術の視力予後も一定していない。今回，著者らは視力障害を訴える12歳男子で，右視神経圧迫を起こしたfibrous dysplasiaに対し右視神経管開放術を試み，良好な視力改善が得られた1症例を報告した。

　視力障害，視野欠損を呈する疾患の中では，fibrous dysplasiaは極めて少ないものであるが，著者らの経験と文献的症例を検討した結果，視機能検査，単純頭部X線写真，CTなどの手段により，fibrous dysplasiaによる視神経圧迫所見の早期発見，早期治療が，視力予後の上で非常に重要なことであると考えられた。

A 12-year-old boy developed fibrous dysplasia involving bilateral optic canals. The visual acuity in the left eye strated to deteriorate 7 months before and was reduced to light perception at our initial examination. X-ray studies showed constriction of the left optic canal and, hence, compression of the optic nerve. Three months later, the visual acuity in the right eye started to deteriorate in a similar man-ner. Surgical decompression of the right optic canal was performed through transfrontal approach 7 weeks after the onset of clinical signs in the right eye.
