留置針を用いた球後麻酔—その2この方法の限界

A retained retrobulbar catheter for repetitive injection of anesthetics during ophthalmic surgery:Part II Limit of this method

　球後針を留置することにより局所麻酔薬を繰返し注入することが可能となり，長時間におよぶ局所麻酔による眼科手術が可能となる。この方法による手術例が35例に達した。35例中10例で術中痛みを訴え，これを完全に除去することができなかった。そのいずれもが眼科手術をすでに受けた症例であり，こうした症例に対する長時間手術にはこの方法にても限界があると考えられた。35例中1例で不幸にして球後針の刺入時に血管損傷をおこし，大量出血を来したが留置針を通して出血が排除されこの方法の長所の一つと考えられた。

For repetitive delivery of local anesthetics during opthalmic surgery, an Angiocath was retrobulbarly inserted and retained. This method resulted in ex-tension of the duration of retrobulbar anesthesia with xylocaine. We employed this procedure in 35 patients during the foredoing 19 months.

Ten patients complained of mild to severe pain during surgery even after additional deliveries of local anesthetics. As a common feature, this group of patients had already undergone ophthalmic sur-gery in the same eye under local or general anes-thesia.
