　人工水晶体挿入に際して最大の問題の一つは，角膜内皮に与える障害と考えられる。

　Binkhorst 2 Loop Lens,Shearing Lens,Iris Medallion Lens （BH2LL, SL, IMLと略す）が挿入された各眼の角膜を，Specula microscopeでその障害程度を観察し，同時に嚢内摘出術および嚢外摘出術による障害との比較を行った。

　片眼白内障手術を受けた患者を，嚢内摘出，嚢外摘出，BH2LL, SL, IMLの5群に分け，おのおのの手術眼と非術眼について，甲南社製スペキュラーマイクロスコープで角膜内皮を撮影し，NEC社PC 8001 MicrocomputerとApple社製Digitizerを使用して，角膜中央部内皮細胞の平均面積を測定した。撮影時期は，術後11〜18ヵ月，次いで術眼と非術眼間の平均面積比（面積増大率）を計算し，各群間でF-test後，t検定により有意差判定を行った。また，平均面積から換算して細胞密度と密度減少率を算出した。

　各術眼の角膜内皮細胞平均面積（（）内は非術眼）および面積増大率：1）嚢内摘出（n＝19）488μm²（369μm²）1,3209,2）嚢外摘出（n＝20）470μm²（341μm²）1,4012,3） BH2LL （n＝21）515μm²（365μm²）1,4285,4） SL （n＝20）476μm²（381μm²）　1,2626,5） IML （n＝15）。

The state of the corneal endothelium was evalu-ated by means of specular microscopy 11 to 18 months after cataract surgery with or without in-traocular lens implantations. Surgery had been performed in one eye and the fellow eye served as control. The cases were divided into 5 groups : in-tracapsular cataract extraction (ICCE) (19 eyes), extracapsular cataract extraction (ECCE) (20 eyes), Binkhorst iridocapsular lens implantation fol-lowing ICCE (21 eyes), Shearing lens implantation following ICCE (20 eyes) and Iris Medallion lens implantation following ICCE (15 eyes).
