緑内障のある白内障眼への眼内レンズ挿入術—角膜内皮の変化と術後炎症

Posterior chamber lens implantation in patients with glaucoma.Evaluation of postoperative inflammation and corneal endothelial cell change

　緑内障67眼に対して後房レンズ挿入術を行い，術後前房内炎症をレーザーフレアセルメータを用いて評価し，術前後の角膜中央部の内皮細胞をスペキュラーマイクロスコープを用いて評価した。術後の前房内炎症は正常30眼と比較して緑内障眼で強く，原発開放隅角緑内障（POAG）36眼と原発閉塞隅角緑内障（PACG）31眼ではPACG眼でより強い前房内炎症が観察された。角膜内皮細胞の減少率は，POAG眼よりPACG眼でより大きかった。PACG眼の中で，急性緑内障発作や濾過手術の既往のある症例には術前から角膜内皮数が少ないものがあり，こうした症例では，手術適応と術後経過観察に注意が必要と考えられた。

Posterior chamber lenses were implanted in 31 eyes of 25 primary angle closure glaucoma (PACG) patients and 36 eyes of 27 primary open angle glaucoma (POAG) patients. Postoperative inflam-mation in anterior chamber was evaluated by laser cell-flare-meter. Corneal endothelial cells wereevaluated by specular microscope. In glaucoma patients post-operative inflammation was greater than in control patients, and in PACG patients it was greater than in POAG patients. The reduction rate of corneal endothelial cell was greater in PACG patients than in POAG patients.Some glau-coma patients who had undergone acute glaucoma attack or glaucoma surgery showed lesser corneal endothelial cells than control patients preoper-atively. In these patients careful consideration is necessary in the postoperative observation.
