眼内レンズ挿入術後にみられた“in the bag”hyphaemaの1例

A case of in-the-bag hyphaema after posterior chamber lens implantation

　嚢間白内障手術後，前嚢後嚢よりなる袋，いわゆる“in the bag （嚢内）”に貯留した前房出血の症例を経験した。

　症例は，64歳の女性で内科的疾患はなく，1986年12月2日老人性白内障で嚢間白内障手術の手法（水晶体上皮細胞を除去，前嚢は除去せず保存。）で後房レンズを挿入した。術後3日目に眼内レンズと前嚢上にまで拡がる前房出血を認め，術後8日目には眼内レンズと後嚢間のみに限局して貯留し鏡面を形成し，その消失までに約3.5か月を要した。“in the bag”hyphaemaは血液が流入貯留しやすい嚢間白内障手術の前嚢切開の型に負う所が大きい。またその自然消失に長期間を要する。“in the bag”の中は，赤血球処理機能の作用しにくい場所であると考えられた。

A 64-year-old female underwent posterior cham-ber lens implantation for senile cataract. We used an intercapsular technique without removal of the anterior capsule. Hyphaema developed 3 days after surgery and extended onto the intraocular lens and the intact anterior capsule. The blood accumulatedin the capsular bag forming a niveau between the intraocular lens and the posterior capsule. The hyphaema disappeared 100 days later following topical anticoagulant application.

Persistence of the capsular sack with a large upper anterior capsulotomy seemed to have result-ed in this unusual postoperative complication.
