シェーグレン病の眼科的治療に関する研究—第1報涙液膜のbreak-up timeおよび粘性剤点眼液の影響

Studies on ophthalmic treatment for Sjögren's disease.:Part 1. Break-up time of the precorneal tear film and effects of mucous substance in artificial tears

　シェーグレン病における乾性角結膜炎の治療を目的として，粘性剤を含有する人工涙液を作製した。作製にあたり，患者の涙液膜を安定に保つため，最適粘性剤濃度を検討した。0％，0.1％，0.3％，0.4％　hydroxyethyl celluloseおよび1％，3％chondroi—tin sulfateを点眼し，その前後の涙液膜のbreak-up timeを測定して検討を行った。従来のbreak-up time測定法では，涙液量の少ない乾性角結膜炎患者の涙液膜に影響が大きいため，必要最少量のfluorcsceinを使用する変法を用いた。

　患者のbreak-up timeは正常者に比し有意に短縮していた。点眼後のbreak-up timeは粘性剤濃度の高いものほど延長していたが，0.3％と0.4％のhydroxycthyl celluloseでは有意差がなかった。3％chondroitin sulfate，点眼後は特に強く延長し，正常者と有意差が認められなかった。

　シェーグレン病の乾性角結膜炎患者の涙液膜を安定に保つため，人工涙液には適量の粘性剤を含有させる必要がある。

I developed a new artificial tear fluid for the treatment of keratoconjunctivitis sicca in Sjogren's disease. I tried to determine the optimum concen-tration of mucous substance in the artificial tear fluid which would best stabilize the precorneal tear film in the patients. Break-up time of tear film was measured before and after instillation of solution containing hydroxyethyl cellulose (0%, 0.1%, 0.3% or 0.4%) or chondroitin sulfate (1% or 3%).
