シエーグレン病の眼科的治療に関する研究—第2報涙液のpHおよび点眼液の影響

Studies on ophthalmic treatment for Sjögren's disease.:Part 2. Tear pH values in patients and effects of artificial tears

　シェーグレン病における乾性角結膜炎の治療を目的として，患者の涙液pHに対する点眼液の影響を検討した。pHの測定には微小複合ガラス電極を直接下結膜嚢内に挿入する方法を用いた。点眼液は3％chondroitin sulfate （pH 5.14）および0.3％hydro—xyethyl cellulose （pH 7.22）を使用し，患者および正常者の涙液pHを点眼前，点眼1分後，5分後に測定した。

　患者の涙液pHは7.62±0.33と，正常持の7.32±0.17に比べ有意にアルカリ側に傾いていた。2種の点眼液使用後の涙液pHは同様の傾向を示した。患者の涙液pHは点眼1分後，5分後共に有意に低下していた。正常音では1分後有意に低下していたが，5分後には有意差がなかった。

　シェーグレン病患者の涙液はアルカリ側に傾いているが，pH 7.22の0.3％hydroxyethyl　celluloseで十分低下させることができる。したがって患者眼に対し刺激がなく，涙液リゾチームへの影響が少ない中性の粘性剤を含んだ点眼液が適当である。

The pH values of the tear fluid were evaluated in 42 patients with keratoconjunctivitis sicca (Sjö-gren) and in 31 normal subjects. The measurement was done by immersing the tip of microelectrode in the tear fluid in the inferior cul-de-sac. The pH values were 7.62±0.33 in the patients and 7.32±0.17 in the normal subjects. The values in the pa-tients were significantly higher than in the normal subjects.

The effects of artificial tears on the pH of the tear fluid were evaluated using 0.3% hydroxyethyl cel-lulose (pH 7.22) or 3% chondroitin sulfate (pH 5.14).
